 A familiar spirit is a demon that serves an individual, like a medium or necromancer. They can ask questions of the dead person that are personal in nature and the demons that were around the person at the time they were alive can answer the questions. It is a deception because they are not really talking to the actual person that has died, but instead they are communicating with a familiar spirit.
